Student's residence card

1. Completed electronic application in MOS 2.0.
2. Digital photograph for submission via MOS 2.0.
3. Scan or photos of all pages of a valid passport.
4. Proof of payment of the stamp duty fee for application processing.
5. Document from the educational institution confirming admission, continuation of studies, or student status.
6. Document from the educational institution specifying the amount of tuition fees per year or semester.
7. Proof of tuition payment — if studies are paid.
8. Documents confirming health insurance coverage in Poland.
9. Documents confirming sufficient financial means for living and studying.
10. Proof of residence address in Poland — if required in the case.
11. Electronic annex from the educational institution to the application — if required for your case in MOS.
12. Translations into Polish — if documents are not in Polish and require certified translation.
13. If submitted by a representative — power of attorney (pełnomocnictwo) and proof of payment of the PLN 17 authorization fee.
14. Electronically certified scans of paper originals — if additional documents need to be submitted and they exist only in paper form.

After the launch of MOS 2.0, paper supplements are not accepted at the service desk. If the voivodeship office requests additional documents, they must be submitted electronically. If a document exists only as a paper original, a scan certified electronically as being true to the original should be prepared in advance.
